Hopefully, there will be no need for any Warranty Service Request on your new "Live Well Custom Home" Home.
However, in the event that you should require warranty service, please use this form.Your concerns should be listed in numerical order.
Please list your name, address, lot number, and daytime phone number during normal business hours where Live Well Custom Homes personnel can reach you.

Once we recieve your written request for service, a Live Well Custom Homes representative will make an appointment to meet with you.At that time, a complete visual and written inspection report reguarding your concerns will be completed.
Following a review of the inspection report, a position letter will then be mailed to you and a repair will be scheduled accordingly.
